Introduction to Thyroid Nodules

Thyroid nodules are abnormal lumps in the thyroid. The condition is very common worldwide and affects more women than men. Many countries, including Singapore and other Asian countries, have reported rising cases of thyroid nodules in part due to advancements in diagnostic techniques. Fortunately, about 95 percent of thyroid nodules are non-cancerous.

Still, early detection and accurate diagnosis play an important role in preventing disease progression. Modern imaging techniques such as ultrasound are especially crucial in evaluating thyroid nodules leading to improved outcomes. If you are concerned about a lump in your neck, talk to Dr Manish Taneja, a trusted specialist thyroid nodule specialist at the Supreme Vascular and Interventional Clinic, for accurate thyroid nodule diagnosis and treatment.

What are Thyroid Nodules?

Thyroid nodules refer to an abnormal growth of thyroid cells, characterized by lumps (nodules) in your thyroid gland. These lumps come in various forms and can either be solid or filled with fluid.

While most thyroid nodules are benign (non-cancerous), a small percentage can turn out to be malignant (cancerous), hence the need for prompt evaluation and treatment. Some symptoms to watch out for include a lump in the neck, difficulty swallowing, hoarseness, hyperthyroidism, and heat intolerance. Still, many nodules are asymptomatic and are only discovered incidentally.

Why Ultrasound is the First Step in Evaluation

A thyroid ultrasound is the primary imaging test used in the initial detection and evaluation of thyroid nodules. Besides being a safe, painless, noninvasive imaging technique, a thyroid ultrasound provides highly accurate images of your thyroid gland, allowing your doctor to determine the size, shape, vascularity, and internal structure of the nodules.

Doctors also use ultrasound to check for calcifications, determine the number of nodules present, or as a guide when performing fine-needle aspiration (FNA) biopsy, another common diagnostic test used to determine malignancy.

Experience and expertise are a crucial part of evaluation to ensure proper interpretation, determine risk levels, and guide therapy.

Thyroid Ultrasound Guidelines: When and Why to Get Scanned

With thyroid ultrasound being a key tool for the evaluation of nodules, professional societies and groups such as the American Thyroid Association (ATA) and the European Thyroid Association (ETA) recommend the test in certain specific situations such as:

  • Newly discovered thyroid lump
  • Family history of thyroid cancer
  • Rapidly growing or symptomatic nodules
  • Abnormal thyroid function tests
  • Follow-up of previously identified nodules

Even when a thyroid ultrasound shows the presence of nodules, only a small percentage will require a biopsy to determine if they are cancerous. This is because some small malignant nodules (less than 1 cm) may remain nonaggressive even without intervention.

For this reason, professional societies such as the American College of Radiology through the Thyroid Imaging, Reporting and Data System (ACR-TIRADS) committee, recommend biopsy based on a risk stratification system. For instance, ACR-TIRADS recommends FNA for nodules 1 cm or larger.

What Happens After the Ultrasound?

Following your ultrasound, the medical team reviews the images and categorizes the nodules based on the risk level. For instance, using the TIRADS classification system, nodules are scored 1 to 5 with higher scores indicating a higher risk of malignancy.

TIRADS 1 represents benign nodules that do not require biopsy. TIRADS 2 represents non-suspicious nodules that do not require biopsy as well.  TIRADS 3 nodules are deemed low suspicion and biopsy is recommended if the nodule is 2.5 cm or greater, or follow-up if the nodule is equal to or greater than 1.5 cm.

TIRADS 4 represents moderately suspicious nodules with biopsy being recommended if the nodule is 1.5 cm or greater and follow-up if the nodule is equal to or greater than 1 cm.  The last category, TIRADS 5, represents highly suspicious nodules, and biopsy is recommended for nodules 1 cm or greater, with follow-up being recommended for nodules 0.5 cm or greater.

In each case, thyroid nodule treatment or monitoring decisions are made based on the ultrasound features. Fortunately, many nodules do not require surgery and can be monitored.

Thyroid Nodule Radiofrequency Ablation (RFA) Treatment

Radiofrequency ablation aims to give patients a non-surgical option for treating thyroid nodules. This procedure is a minimally invasive option that a surgeon can perform, should the preliminary tests determine that you would benefit from it.

The Advantages of RFA of Thyroid Nodules

The major advantages of the technique are as below.

  1. It is done as day surgery procedure under light sedation.
  2. It takes about 45 minutes to one hour to complete the entire procedure.
  3. Patients are discharged the same day.
  4. Normal thyroid tissue adjacent to the nodule is preserved.
  5. It does not affect thyroid function.
  6. Multiple nodules can be treated at same time.
  7. The treated nodules do not regrow again.
  8. No surgical scar with needle mark on the skin.
  9. Quick recovery and back to full activity and normal lifestyle.
  10. The technique is suitable to treat even large thyroid nodules.

Caring For Your Thyroid Health

Have you ever experienced difficulty swallowing or breathing? You may have risk factors including a family history of thyroid nodules or iodine deficiency. Thyroid nodules are abnormal lumps (growths) within the thyroid gland, a butterfly-shaped gland located below your voice box. Even if most thyroid nodules are benign, 10-15% of nodules are cancerous. Sometimes overgrowth of normal thyroid tissue can lead to hyperthyroidism. In other cases, multiple nodules can form in the thyroid called multinodular goiter. Fluid filled thyroid cysts can be one type of thyroid nodule within the thyroid. More complex nodules may lead to thyroid cancers. Depending on the root cause and type of nodule you have, patients can monitor with regular ultrasound or even have a thyroid fine needle aspiration biopsy (FNAC) if required. Surgery to remove partial or whole thyroid gland can be done but thanks to minimally invasive treatments such as Radiofrequency Ablation (RFA), this procedure can treat noncancerous and cancerous thyroid lumps.
