Basic Thyroid Nodules Definitions and Medical Terminology

  • Thyroid Nodule: Thyroid nodules are abnormal lumps (growths) within the thyroid gland. This is a very common condition seen in approximately 30% of adults.

  • Thyroid Cyst: Thyroid cysts are nodules that form within the thyroid. There are two types of thyroid cysts – fluid-filled and complex.

  • Thyroid Gland: The thyroid gland is a butterfly shaped gland in the neck just below the voice box. Thyroid gland produces hormone that is required for our metabolism with under or over hormone secretion both causing physical problems.

  • Multinodular Goiter: A multinodular goiter is a thyroid gland that has grown larger than usual and developed several small, rounded bumps or nodules.

  • Lymph Node: Lymph nodes are small-bean-shaped organs critical to your body’s immune system. They can be anywhere from a few millimeters to up to two centers in diameter. Lymph nodes in your neck are cervical lymph nodes. Anterior lymph nodes are in the front of your neck.

  • Cold Thyroid Nodule: Cold thyroid nodules are lumps that are generally unable to produce excess thyroid hormones and produce no symptoms.
  • Hot Thyroid Nodule: Hot thyroid nodules are thyroid nodules that produce excess thyroid hormones. This hormone imbalance can lead to hyperthyroidism and other regulatory problems.

Thyroid Nodules Diagnostic Terms

  • Thyroid Fine Needle Aspiration Biopsy (FNAC): FNAC is a simple procedure and biopsy used to detect thyroid nodules cancer and is performed with local anesthetic. Ultrasound will be used to obtain images that guide the doctor to the nodule. The doctor will insert a very fine thin needle, even smaller than ones used for blood draws, into the nodule to draw out cells.

  • Thyroid Nodules Ultrasound Scan: When a thyroid nodule is discovered, it needs to be evaluated. The key initial tool is ultrasound imaging. It is fast, accurate, and non-invasive. Ultrasound imaging is used for initial evaluation.

  • Thyroid Function Tests: These are blood tests to measure levels of TSH, T3, and T4 hormones.

Thyroid Nodules Treatment Options in Singapore

  • Thyroidectomy: After you complete the diagnostic process, the thyroid professional you visit might determine that you need a surgical intervention to deal with your nodules. In this case, you’ll likely undergo a thyroidectomy. A thyroidectomy is a surgical procedure aimed at removing all or part of the thyroid gland, often necessary for treating thyroid cancer, hyperthyroidism, or large thyroid nodules.

  • Radiofrequency Ablation (RFA): One of the most common ablation techniques is radiofrequency ablation. RFA is a minimally invasive procedure used to treat benign thyroid nodules. It involves inserting a thin needle-like probe into the nodule, which emits high-frequency electrical currents. These currents generate heat, which destroys the targeted thyroid tissue, causing the nodule to shrink over time. RFA is an effective alternative for patients who do not want surgery and have nodules causing symptoms.

  • Monitoring: If your thyroid nodules are relatively noninvasive, then they’ll likely suggest the active monitoring approach to you. This means that you’ll both be carefully watching your nodules, to ensure that they don’t change or cause you more discomfort in the future.

Medications and Management

  • Medicine for Thyroid Nodules Treatment: Most thyroid nodules are benign, but some can be toxic or cancerous. New treatment for thyroid nodules depends on the expert diagnosis and thyroid nodule condition. In most cases, your physician can treat thyroid nodules without surgery. The fact is most thyroid nodules are non functioning and there is no medication or medical treatment for them. Read our blog post, 'Debunking Common Myths About Medications for Thyroid Nodules Treatment', to learn more. These include:
  1. Antithyroid medications
  2. Radioactive iodine
  3. Beta-blockers
  4. Thyroid Hormone
  5. Targeted drugs

Specialist

  • Neurointervention specialist: A doctor with specialized training in the use of minimally invasive, image-guided techniques to treat complex conditions of the brain, neck, and spine.
  • Endicronologist: A specialist in diagnosing and treating disorders of the endocrine system, which includes glands that produce hormones regulating metabolism, growth, development, and reproduction.

Associated Conditions

  • Hyperthyroidism: Hyperthyroidism, or an overactive thyroid, can cause a rapid heart rate, high blood pressure, and increased risk of irregular heart rhythms. Also known as thyrotoxicosis, this overactivity of the thyroid gland results in overproduction of the thyroxine (T4) hormone.
  • Hypothyroidism: Hypothyroidism, or an underactive thyroid, can lead to high cholesterol levels, increased risk of heart disease, and fluid buildup around the heart. It is a condition whereby the thyroid fails to produce enough thyroid hormones (triiodothyronine (T3) and thyroxine (T4)). The body requires an adequate supply of these hormones to function optimally, without which many key functions are slowed down.
  • Grave's Disease: Grave's disease is an autoimmune disorder that affects the thyroid gland, leading to an overproduction of thyroid hormones, a condition known as hyperthyroidism. In this condition, the immune system mistakenly attacks the thyroid gland, causing it to become enlarged and overactive. Sudden weight loss, bulging eyes, trouble toleraing heat, and an increased heartbeat are some of the symptoms of an autoimmune disorder known as Graves disease. Severe Graves has some life-threating complications.
  • Hashimoto's Disease: Hashimoto's disease or thyroiditis, is an autoimmune disorder in which the immune system attacks the thyroid gland, leading to chronic inflammation and often resulting in an underactive thyroid (hypothyroidism). This condition is the most common cause of hypothyroidism. Symptoms may include fatigue, weight gain, sensitivity to cold, constipation, dry skin, hair loss, and depression. The condition is diagnosed through blood tests that measure thyroid hormone levels and thyroid antibodies.

Caring For Your Thyroid Health

Have you ever experienced difficulty swallowing or breathing? You may have risk factors including a family history of thyroid nodules or iodine deficiency. Thyroid nodules are abnormal lumps (growths) within the thyroid gland, a butterfly-shaped gland located below your voice box. Even if most thyroid nodules are benign, 10-15% of nodules are cancerous. Sometimes overgrowth of normal thyroid tissue can lead to hyperthyroidism. In other cases, multiple nodules can form in the thyroid called multinodular goiter. Fluid filled thyroid cysts can be one type of thyroid nodule within the thyroid. More complex nodules may lead to thyroid cancers. Depending on the root cause and type of nodule you have, patients can monitor with regular ultrasound or even have a thyroid fine needle aspiration biopsy (FNAC) if required. Surgery to remove partial or whole thyroid gland can be done but thanks to minimally invasive treatments such as Radiofrequency Ablation (RFA), this procedure can treat noncancerous and cancerous thyroid lumps.
