Overview

Blood circulation to the legs involves two systems broadly. First is the arterial system taking blood to the legs from the heart carrying oxygenated blood. Second is the venous system taking deoxygenated blood from the legs to the heart. This venous system comprises two parts: superficial venous system under the skin and deep venous system next to the bones and the deep muscles. Deep venous system carries majority of the blood from the legs to the heart.

Deep vein thrombosis or DVT as it is commonly known a (DVT) is a condition in which blood clots form in the deep veins of the lower limbs. Uncommonly deep veins of the arm can also get affected but this is very rare and usually under special circumstances. Most patients with DVT are concerning requiring urgent treatment but not life-threatening.

In some patients especially if not recognised in time or untreated; these clots can progress towards the heart. These clots can break off in pieces and travel into the heart and lungs resulting in a more severe condition known as Pulmonary embolism (PE). Some patients with pulmonary embolism have life-threatening clots preventing oxygen transfer from venous blood posing a significant risk of death.

What is Venous Thromboembolism?

Venous Thromboembolism (VTE) is a condition that includes two related but distinct medical issues: Deep Vein Thrombosis (DVT) and Pulmonary Embolism (PE). VTE occurs when a blood clot forms in the deep veins, often in the legs (DVT), and can potentially break loose, travel through the bloodstream, and lodge in the lungs (PE), causing a blockage in the pulmonary arteries. VTE is a serious and potentially life-threatening condition that requires prompt medical attention to prevent complications and improve outcomes. In more severe cases, DVT can progress to the following conditions:

  1. Deep Vein Thrombosis (DVT): A condition where a blood clot forms in a deep vein, usually in the legs, which can lead to serious complications if the clot dislodges and travels to the lungs.
  2. Deep Vein Thrombosis with Pulmonary Embolism: A severe complication of DVT where the blood clot travels to the lungs, causing a potentially life-threatening blockage in the pulmonary arteries.
  3. Pulmonary Embolism (PE): A serious condition where one or more arteries in the lungs become blocked by a blood clot, typically originating from a deep vein thrombosis in the legs.
  4. Phlegmasia Alba Dolens: A rare and severe form of deep vein thrombosis characterized by painful swelling and whitening of the leg due to the blockage of major veins.
  5. Phlegmasia Cerulea Dolens: A life-threatening progression of deep vein thrombosis, causing severe pain, swelling, and a blue discoloration of the leg due to the near-total blockage of venous blood flow.

Venous Thromboembolism Risk Factors

  • Surgery
  • Trauma (Major or lower extremity)
  • Immobility
  • Malignancy
  • Cancer
  • Previous DVT or PE
  • Increasing age
  • Pregnancy and post-partum period
  • Estrogen containing oral and hormone replacement therapy
  • Medical conditions like AMI, IBD, Nephrotic syndrome
  • Obesity
  • Smoking
  • Varicose veins
  • Thrombophilia (inherited or acquired)

Symptoms of Deep Vein Thrombosis

Symptoms can be variable depending on the underlying extent or location of the clot

  • Some patients may have minimal symptoms like mild swelling or aching or discomfort or cramps in the calf or whole leg
  • Swollen and tender legs: The swelling and tenderness may start in the calf and then extend towards the thigh and groin
  • Swelling of legs with redness or prominent visible blue veins under the skin
  • Fast heartbeat (tachycardia)

Shortness of breath either in isolation or related to the limb swelling: This is a sign that the patient needs urgent and medical attention, as they may be experiencing a pulmonary embolism.

Deep Vein Thrombosis Risk Factors

  • Long periods of immobility (e.g. long-haul flights or travel journeys): this is one of the most well known risk factor for deep vein thrombosis
  • After major surgery involving the limbs, pelvis or abdomen
  • Dehydration
  • Major trauma to the arms/legs
  • Having a genetic predisposition to blood clotting, causing DVT
  • Abnormal blood vessel anatomy (e.g. May-Thurner syndrome or Paget-Schroetter Syndrome)
  • The presence of underlying cancer which can result in the blood being more prone to clotting (paraneoplastic syndrome)

How Is a Deep Vein Thrombosis Diagnosed?

Diagnosis is based on clinical assessment that often includes risk factor history, blood test and ultrasound scan showing the presence of clots in the deep veins. Other associated investigations include a CT scan of the leg veins and lung arteries.

 

  • Duplex Ultrasound of the leg
  • Elevated d-dimer
  • Anticoagulation 

Deep Vein Thrombosis Treatment Options

  • Medical management: The most common and standard treatment suitable for most patient is blood thinning medication (anti-coagulation medication )
  • Minimally invasive pharmaco-mechanical treatments for DVT: This treatment is catheter based treatment requiring access to veins in the legs using tiny incisions less than a centimeter. The indication for this treatment is if there is lack of response to medical treatment, if the DVT is extensive (e.g. extending up the thigh and into the pelvic veins) or is life-threatening. Most clots are removed using this technique immediately and anticoagulation medication are continued at same time preventing new clots .The clots from the lungs can also be removed at the same time if required using these techniques. 
  • Minimally invasive interventional treatment are advantageous and depends on the patient’s conditions.
    – It can be extremely useful in a subgroup of patients.
    – Catheter based techniques are used to clear the clot.
    – This will take around 90 minutes to do with about 1-2% risk.
    – It is complementary to anticoagulation therapy.

What is Venous Compression Syndrome?

Venous Compression Syndrome refers to a group of conditions in which veins are compressed by surrounding structures, such as arteries, bones, or muscles, leading to restricted blood flow and increased risk of blood clots. This compression can occur in various parts of the body.

 

  1. Paget-Schroetter Syndrome – Thoracic Outlet:
    A condition also known as “effort thrombosis,” where a blood clot forms in the veins of the upper arm or shoulder due to compression of the veins at the thoracic outlet, often associated with repetitive arm movements.
  2. May-Thurner Syndrome – Pelvis:
    A vascular condition where the left iliac vein is compressed by the right iliac artery, leading to an increased risk of deep vein thrombosis (DVT) in the pelvis and left leg.
  3. Nutcracker Syndrome – Renal:
    A rare condition where the left renal vein is compressed between the abdominal aorta and the superior mesenteric artery, causing blood flow obstruction and symptoms such as hematuria (blood in urine) and flank pain.
  4. Popliteal Vein Compression:
    A condition where the popliteal vein, located behind the knee, becomes compressed, often by surrounding muscles or structures, leading to restricted blood flow and an increased risk of deep vein thrombosis (DVT).

Left Common Iliac Vein Compression Syndrome

This is commonly called May-Thurner Syndrome also known as Iliocaval compression syndrome or Cockett’s syndrome. It results in the compression of the left common iliac vein between the right common iliac artery and the spine.

It can present as:

  • Reccurrent left lower limb swelling
  • Heaviness and pain
  • Acute ilio-femoral DVT and pulmonary embolus
  • Phlegmasia

Post Thrombotic Syndrome

About one third of patients with acute deep vein thrombosis develop post-thrombotic syndrome.

 

  • 5-10% will develop severe post-thrombotic syndrome.
  • It also predisposes to recurrent deep vein thrombosis.
  • Persistent leg pain and swelling in severe cases with nonhealing ulcers and venous dermatitis.

    When should Deep Vein Thrombosis be treated urgently?

    Deep vein thrombosis requires prompt medical assessment because untreated blood clots may increase the risk of serious complications, including pulmonary embolism.

    Some patients require urgent hospital-based care, while others can be safely managed with outpatient treatment. The most appropriate approach depends on:

    • The location of the blood clot

    • Current medications and bleeding risk

    This comprehensive assessment helps determine whether anticoagulation alone is appropriate or whether additional intervention should be considered.

    How do specialists decide which DVT treatment is right for you?

    Treatment is individualised based on careful clinical assessment.

    What are Deep Vein Thrombosis (DVT) treatment options?

    Once deep vein thrombosis has been confirmed, the next step is deciding which treatment approach is most appropriate. The goal is to prevent the blood clot from enlarging, reduce the risk of pulmonary embolism, preserve healthy vein function, and minimise long-term complications.

    The recommended treatment depends on several factors, including the location and extent of the clot, the severity of your symptoms, your overall health, and how you respond to initial treatment.

    Medical Management

    For most patients, the standard treatment for deep vein thrombosis is anticoagulation (blood-thinning medication). These medications help prevent the clot from growing and reduce the risk of new clots forming while allowing the body to naturally break down the existing clot over time.

    Many patients recover successfully with anticoagulation alone, supported by regular follow-up and ongoing monitoring.

    Minimally Invasive Catheter-Based Treatment

    For selected patients, minimally invasive intervention may offer additional benefits beyond medical therapy alone.

    Catheter-based treatment may be considered when:

    • The blood clot is extensive, particularly if it extends into the thigh or pelvic veins.
    • Symptoms remain severe despite anticoagulation.
    • There is concern about preserving long-term vein function.
    • The clot is associated with life-threatening complications, such as significant pulmonary embolism in carefully selected cases.

    Using tiny skin incisions, specialised catheters are guided into the affected vein to deliver treatment directly to the clot. Depending on the clinical situation, this may involve clot-dissolving medication (pharmacological thrombolysis), mechanical clot removal (mechanical thrombectomy), or a combination of both techniques (pharmacomechanical thrombectomy).

    In many cases, a large proportion of the clot can be removed during the procedure, helping to restore blood flow while anticoagulation therapy continues to reduce the risk of further clot formation.

    Minimally invasive treatment is not necessary for every patient with DVT. Instead, the decision is individualised after careful assessment of your condition.

    • Managing ongoing leg swelling
    • Guidance on returning to normal activities
    • Compression therapy if recommended
    • Assessment for post-thrombotic syndrome
    • Review of underlying clotting risk factors

    The aim is not only to treat the current clot but also to reduce the likelihood of future blood clots and preserve healthy vein function.

    How do I reduce the risk of future blood clots?

    After recovering from DVT, your specialist will discuss strategies to reduce the risk of recurrence.

    Depending on your individual circumstances, this may include:

    • Remaining physically active
    • Walking regularly during long-distance travel
    • Maintaining a healthy weight
    • Staying well hydrated
    • Managing chronic medical conditions

    • Taking anticoagulant medication for the recommended duration
    • Attending regular follow-up appointments
