Overview
Cerebral venous thrombosis (CVT) is less common known cause of acute stroke in young patients.
Stroke is commonly caused by clots in arterial circulation supplying blood from heart to the brain that block blood supply with damage to the brain cells.
In some patients, these clots form in the venous circulation affecting blood vessels that take blood from the brain back to the heart. These clots prevent blood from clearing from the brain circulation with significant potential risk of bleeding inside the brain causing stroke and death.
Recognising Symptoms
CVT is commonly seen in young patients. They can present with severe headaches , focal neurological deficits, altered conscious state or coma depending on location and extent of clot with associated brain damage.
Causes and Risk Factors
Most commonly its affects young females. Important risk factors for CVT are oestrogen-containing oral contraceptives, prothrombotic (hypercoagulable) conditions (genetic or acquired thrombophilias), pregnancy and the puerperium, infections, malignancy, 14 head injury (causing direct trauma to venous structures) and inflammatory diseases. There has also association described recently after vaccination.
Diagnosis and treatment of Cerebral Venous Thrombosiss
CVT is diagnosed on MRI/MRV scan of the brain with a high degree of accuracy.
Most patients are managed with anticoagulant medication. Occasionally catheter based techniques are used to remove the clots from the brain in patients with severe life threatening clinical presentation not responding to medical therapy. In selected patients with severe or worsening symptoms, catheter-based neurointerventional procedures may occasionally be considered to help remove or treat the clot. Very rarely surgery is needed in patients with critical bleeding in the brain.
